Do you have Type 2 diabetes? Are you not using insulin?

TeleCARE Manitoba is a new program that will help you learn more about diabetes to help you live a healthier life.

You will be able to talk directly to a specially trained nurse and dietitian, by telephone from the comfort of your home.

If you do not have access to a telephone, a videoconference service is available through MBTelehealth. Videoconferencing allows people in different places to see and hear each other at the same time on a TV screen. Talk to a health-care provider, such as your doctor or nurse, if you would like to use MBTelehealth.

Many people have benefitted

Those who have joined the program have said it is helpful to continue learning about their health and how to improve it.

"The program keeps me in touch with my health, keeps me more alert about symptoms and how I should respond," said a 52-year-old participant.

Another participant said, "This program has given me the tools I needed to lose 60 pounds, which has helped me to be able to walk five miles per day... I have never felt so good!"

It is easy to join

Joining TeleCARE Manitoba's Type 2 diabetes program is simple.

  1. Sign up by calling 204-788-8688 or toll free 1-866-204-3737. Or, you can ask your health-care provider (example: a doctor or nurse) to sign you up.
  2. Once you have signed up, a nurse will call you to ask questions about your diabetes and your health.
  3. You and the program team will set up times to talk about a diabetes health plan made just for you. You will also get tips on what you can do to be healthier and you can talk about any problems you are having with your diabetes. Through this program, you will learn how to take control of your own care.
  4. The program is at least one year; but, you can choose to leave at any time.

World Food Day

June 16, 2009 - in Cross Lake

This year the Home Care Nurses worked very hard with the ADI worker in Cross Lake to promote World Food Day on October 16, 2009.

Community members were coming in to the community hall for their Flu Vaccination. While they waited around for 15 minutes after their shot, the Cross Lake staff took advantage of this time to teach them a thing or two about healthy eating and diabetes management. They had beautiful stations set up that really caught community members' attention. The Cross Lake staff also provided information to the clients on diabetes, provided insulin administration teaching, showed clients how to monitor their random blood sugar levels, check blood pressure and provide foot care assessments. Although most people were too shy to have their feet inspected in public, everyone seemed to learn a lot and seemed to really enjoy the information provided.
